Description
Sauk Prairie Healthcare is in the early stages of developing our next community health needs assessment and our strategic plan and we need your help. We are conducting a series of focus groups throughout our service area with community leaders and business representatives. Because of your involvement in the community, you were recommended as someone we should ask to participate in one of our focus groups. During the next 60 days, we will be meeting with more than 100 people in small groups to identify high priority needs in your community. We are doing this so we can determine what role Sauk Prairie Healthcare can play in addressing those needs. Can we count on your participation?
